SUMMIT COVID Policy

In accordance with local guidance, all SUMMIT Team members are vaccinated. Additionally, all guests over the age of 12 must present proof that they have received at least one dose of a vaccine authorized by the FDA or WHO. A photo ID showing name and DOB or photo must be presented with proof of vaccination. Guests under 12 must wear a face covering unless eating or drinking when 6' of social distance from others can't be maintained.

Examples of acceptable proof of vaccination include:

• CDC vaccination card (photo or original hard copy)
• NYC COVID Safe App
• New York State Excelsior App
• Official record of a WHO vaccine administered outside the United States (photo or original hard copy)

Examples of acceptable ID:

• Driver’s license
• Government-issued ID
• IDNYC card
• Passport
• School or work ID card
